This Master Services Agreement (this “MSA” or “Agreement”) is made and entered by and between LifeWallet, LLC, a Delaware limited liability company (“LifeWallet”), and Patient/Provider/Authorized HIPAA third-party representative requesting Electronic Health Records through LifeWallet/LifeChain - EHR (“Customer”) for the treatment and care of its patients. LifeWallet and Customer may be referred to herein individually as a “Party” and collectively as the “Parties.” The effective date of this Agreement shall be the date as of the last date fully executed by the Parties (the “Effective Date”).
