(“Agency”) has elected to participate in the Homeless Management Information System (“HMIS”) administered by the Indiana Housing and Community Development Authority (“IHCDA”) and therefore is entering into this HMIS Participation Agreement for Agencies (this “Agreement”). The HMIS is a database and case management system that collects and maintains information on the characteristics and service needs of clients. The system collects and stores client –level data, which can be used to generate unduplicated and aggregate reports to determine the use and effectiveness of the services being provided to the homeless population.
