This Salary Reduction Agreement (the “Agreement”) must be completed by each University of Chicago employee enrolling in the Supplemental Retirement Program (“SRP”) unless the employee enrolled online during the annual open enrollment period. Employees may elect to participate in the SRP by completing this Agreement anytime during the calendar year. The contribution amount specified in Paragraph 2 of this Agreement may be increased or decreased (at the employee’s election) only once during the calendar year. The employee may discontinue SRP contributions anytime by written notice to Benefits. Once discontinued, SRP contributions may be reinstated only once during the calendar year.
