This Data Processing Addendum (“DPA”) is made between Cognito, LLC., a South Carolina corporation (“Cognito Forms”) and Estrella Warbirds Museum (“Customer” or “Controller” or “Estrella Warbirds Museum”) as an addendum to the Cognito Forms Terms of Service (the “Terms of Service”). This DPA is effective as of 6 December 2023 (“Effective Date”), which is the date Estrella Warbirds Museum indicated its acceptance of this DPA electronically. This DPA was electronically signed by Peter Visel, Webmaster, Board Advisor on behalf of Estrella Warbirds Museum on the Effective Date. Terms not otherwise defined below will have the meaning set forth in the Terms of Service.
