This Student Diversity Leadership Conference (“SDLC”) Student/Family Agreement (the “Agreement”) is intended to ensure that the SDLC Code of Conduct is delivered to and seen by all SDLC participants. This is not a registration document. This Agreement should be read and signed by the Student Participant and one of the student’s parents/guardians (“parent(s)”) and submitted to you, the student’s school. All participating schools must keep this signed Agreement on file for a minimum of four (4) years, pursuant to the school’s agreement with the National Association of Independent Schools (“NAIS”).
