This Master Subscription Agreement (“MSA”) is effective as of August 21, 2014, (“Effective Date”), by and between Castlight Health, Inc., a Delaware corporation located at Two Rincon Center, 121 Spear St., Suite 300, San Francisco, CA 94105 (“Castlight”) and Center Grove Community School Corporation, an Indiana school corporation at 4800 W. Stones Crossing Road, Greenwood, IN, 46143 (“Customer”) as sponsor of the health plan components of the Center Grove Community School Corporation Group Benefit Plan.
