This Agreement will form part of the Agreement arrangements between Swanwick Lodge and the Placing Authority which will apply in its entirety. It details the Care and Services provided to _________________ (Name of young person) and where possible must be completed prior to admission, once approval for a placement has been agreed. When the admission is urgent, it should be completed as soon as possible, or in any event within 2 working days. It must be agreed and signed by both parties and appended to the child/young person’s Care Plan & LAC Forms or other authorised documents.
