This CONTRACT EXTENSION AGREEMENT (“Extension”) is dated as of [Insert Date] (the “Effective Date”), by and between: [Insert Party A Name] [Insert Party A Address] AND [Insert Party B Name] [Insert Party B Address] Collectively known as the “Parties”.
