Santa Clara Valley Health and Hospital System (“County”) and NAME OF BUSINESS (“Contractor”). This Agreement shall have an effective date of DATE (“Effective Date”). Physician and County may, from time to time, be referred to in this Agreement individually as either “Party” or collectively as “Parties.”
