Whereas the undersigned Owner has applied for a retirement income fund (the “Contract”) with Concentra Trust (the “Issuer”) to receive and hold those funds in accordance with the Province of Manitoba The Pension Benefits Act (the “Act”) and the Pension Benefits Regulation (the “Regulation”), as both may be amended from time to time.

THIS IS AN ADDENDUM TO AN RRIF CONTRACT BETWEEN:

 A life income fund (LIF) is a registered retirement income fund (RRIF) to which the additional terms and conditions in this addendum apply. Together, this addendum and the RRIF contract to which it is attached form your LIF contract.
