In consideration of being allowed to participate in Activities at River Valley Ranch for the date(s) of I, for myself and any minor children for whom I am parent, legal guardian, or otherwise responsible and for my/our heirs, personal representatives or assigns, hereby acknowledge the risks of injury or damage (to property or personal injury) involved in participating in the paintball activities offered by River Valley Ranch. I further understand that participation in these activities contains risks we each appreciate and voluntarily assume because the minor and we choose to do so. My participation in such activities and/ or use of such equipment may result in my injury or illness including but not limited to bodily injury, disease strains, fractures, partial and/or total paralysis, eye injury, blindness, heat stroke, heart attack, death, or other ailments that could cause serious disability.
