This Specialized Center of Research Grant Agreement (“Agreement”), is entered into on [DATE] (“Effective Date”) between The Leukemia & Lymphoma Society, Inc., a New York not-for- profit corporation, with an office located at 3 International Drive, Suite 200, Rye Brook, New York 10573 (“LLS”), and the Sponsoring Institution set forth below (“Sponsoring Institution”). Under this Agreement, LLS will provide the Sponsoring Institution with the grant set forth below, and Sponsoring Institution will cause the grantee set forth below to perform certain duties. For good and valuable consideration, the receipt and sufficiency of which is hereby acknowledged, the parties agree to the terms of this Agreement, which consists of this cover sheet (“Cover Sheet”), the Terms and Conditions attached hereto as Attachment 1 (“Terms”) and any appendices attached thereto.
