The signatories below, representing specific communities and specific populations within and including the county of _________________________, confirm that they have participated in the development of the county’s CDBG-CV program application and agree that the program, when implemented, will have beneficial impact on their populations to prevent, prepare for, and respond to the COVID-19 pandemic and other infectious diseases.
