The above-named person, having attended a review session and passed a test of knowledge concerning the law and practice of involuntary detention under the Lanterman Petris Short Act, agrees to the following terms and is hereby individually designated as having authority to involuntarily detain and treat pursuant to section 5150 California Welfare and Institutions Code. This individual designation is limited to detentions initiated within the County of Los Angeles and to the designated hospital listed above. This designation expires at the end of the designation period indicated above, unless earlier terminated by either party without cause.
