This Amendment No. 1 (the “Amendment”) is made by and between Verizon North Inc. and Verizon South Inc. (collectively “Verizon”), a Wisconsin corporation with offices at 8001 West Jefferson, Ft. Wayne, IN 46804 and 201 N. Franklin Street, One Tampa City Center, Tampa, FL 33602, respectively, and Insight Phone of Illinois, LLC d/b/a Insight Phone, a Limited Liability Company with offices at 10200 Linn Station Road, Suite 100, Louisville, KY 40223 (“Insight Phone”), and shall be deemed effective on May 29, 2006 and in no event later than the date on which the Agreement (as defined below) takes effect (the “Amendment Effective Date”). Verizon and Insight Phone are hereinafter referred to collectively as the “Parties” and individually as a "Party". This Amendment covers services in Verizon’s service territory in the State of Illinois (the “State”).
