The undersigned (the “Participant”) acknowledged and agrees that the Participant is fully aware of the risks and hazards connected with the recreational, sporting, and all other activities associated with the Participant’s use of the facilities and equipment provided by by SLLC Company, LLC, an Ohio limited liability company (the “Company”), and is fully aware that such activities include the risk of illness, injury, disability or even death. The Participant hereby elects to voluntarily use the facilities and equipment and participate in said activities, knowing that the activities may be hazardous to the Participant’s health and property. The Participant hereby agrees to comply with all rules imposed by the Company for use of the facilities and equipment and hereby agrees to use all facilities and equipment in a manner consistent with its intended design and purpose.
