This Advisory and Amendment (“2021 Amendment”), effective July 1, 2021, modifies certain compensation and other provisions of your AmeriHealth Member Hospital Agreement or Managed Care Hospital Agreement, as may have been amended, (collectively “Provider Agreement”) with AmeriHealth HMO, Inc. and AmeriHealth Insurance Company (collectively referred to herein as “AmeriHealth”).
