I_____________________________ agree to make the following payments to WYHA for the outstanding balance due. I understand that if payments are missed or are late my child will not be allowed to play (No-Pay, No-Play Policy). I also understand that this agreement does not take effect until signed by a member of the WYH Board. A copy of this signed agreement will be sent back to the inquiring family.
