It is agreed by and between the State of Vermont, Department of Vermont Health Access (hereafter called the “State”) and Springfield Medical Care Systems (hereafter called the “Contractor”) that the contract on the subject of providing administration of the Blueprint for Health in the Springfield Health Service Area, effective October 1, 2015, is hereby amended effective March 1, 2017, as follows:
