I, fully understand that my child’s participation in the City of Bishop Recreation Programs (hereinafter "event/program") exposes me to the risk of personal injury, communicable diseases, illnesses, viruses, death or property damage. I hereby acknowledge that I am voluntarily allowing my child, to participate in this event/program and agree to assume any such risks. Events/programs included, but not limited to are: Gymnastics, After School Program, Summer Camp Program, recreational sports (softball, baseball, soccer, basketball, volleyball), swim lessons and water slide use, and Kid’s Night Out.
