This Agreement ( “Agreement”) is made effective this 26 day of July , by and between Lake County Health Department, with a principal place of business at 2900 West 93rd Ave., Crown Point, IN 46307 ("LCHD"), and Purdue University, on behalf of the Purdue University Extension, with a principal place of business at 2291 N. Main Street, Crown Point, Indiana 46307 ("Grantee"), collectively referred to herein as the "Parties."
