The current fitness center fee is $25.00 per month payable in advance on the first day of each calendar month. All initial payments must be made by credit/debit card or check. The undersigned hereby authorizes and agrees that the credit/debit card provided with this application and agreement will be charged for each payment of the then current fee, unless terminated as provided herein.
