This Software as a Service (“SaaS”) Agreement and all Exhibits annexed hereto (the “Agreement”) is made and entered into on (“Contract Start Date”), by and between DICOM Grid, Inc. dba Ambra Health (“Company”), and (“Subscriber”). In consideration of the mutual promises contained herein, the parties hereby agree to the following:
