(hereinafter referred to as DENTIST) who is qualified and licensed to practice Dentistry in the State his or her dental practice is domiciled, and Dominion Dental Services, Inc. and Dominion Dental Services USA, Inc., both Virginia Corporations (hereinafter referred to collectively as PLAN). Whenever mentioned herein, the term DENTIST shall include all employees of DENTIST, all partners, dental associates, and all staff personnel under DENTIST’S direct supervision and/or control.
