This Illinois Amendment to the Local Exchange Telecommunications Services Resale Agreement under Sections 251 and 252 of the Telecommunications Act of 1996 (the “Amendment”) is dated as of December 24, 1999 by and between Ameritech Information Industry Services, a division of Ameritech Services, Inc., with its principal offices at 350 North Orleans, Chicago, Illinois 60654, on behalf of and as an agent of Ameritech Illinois, (“Ameritech”) and Suretel, Inc., with its principal offices at 5 N. McCormick, Oklahoma City, Oklahoma 73127 (“Requesting Carrier”).
