The Health and Human Services Commission (“HHSC” or “System Agency”), an administrative agency within the executive branch of the State of Texas, and Heart of Central Texas Independent Living Center , Inc.(“Grantee” or “Provider”), a nonprofit corporation and having its principle office at 222 E. Central Avenue, Belton, Texas, 76513, each a “Party” and collectively “the Parties”, enter into the following contract for Centers for Independent Living Program (the “Contract”).
