StrataEMR Patient Portal provides a view into your Electronic Medical Record (EMR) which is maintained by your Provider and gives you a secure way to communicate with your Provider. StrataEMR Patient Portal is governed by these Terms of Use (Terms). If you do not agree to these Terms, you may not establish a StrataEMR Patient Portal connection.
