This is an Agreement made and operative as of the 1st day of January, 2024, between the County of Placer, through its Health and Human Services Department, a political subdivision of the State of California, hereinafter referred to as “COUNTY”, and GHC of Lakeside, LLC dba Lakeside Special Care Center a Limited Liability Company, hereinafter referred to as “CONTRACTOR.”
