In the event that the First Aid staff at Hershey Entertainment & Resorts determines, in its sole discretion, that my child or children (hereinafter referred to as “Child”) require(s) additional professional medical attention, I hereby grant permission for Hershey Entertainment & Resorts to transport my Child to any medical facility for medical evaluation or treatment. In the event that my Child is transported, an attempt will be made to notify me by telephone at the emergency contact number provided.
