Senior Ball is a once in a lifetime event and of course student safety is our greatest concern. This agreement is required in the hope that all participants understand the expectations and the concern for safety. Guests who are not Campolindo students must submit this form before a ticket may be purchased. NOTE: If guest is not a high school student, the guest must fill out the guest agreement and get it signed by Campolindo Administration. He or she must provide color copies of their driver’s license and college ID (if applicable). Guests must be under 21 years of age.
