This Independent Provider Agreement (“Agreement”) is made and entered into this 1st day of July, 2022, by and between Glenn County, a political subdivision of the State of California, by and through its Health and Human Services Agency Behavioral Health Division ("County"), and Crestwood Behavioral Health, Inc., (“Provider”).
