This Health Savings Account Enrollment and Contribution Agreement (the “Agreement”) is entered into by and between (the “Employer”), and Optum Bank, Inc., a Utah chartered FDIC insured financial institution (the “Bank”) (collectively, the “Parties”) and shall be effective as of the date of last signature (the “Effective Date”).
