Customer: Date: Work Site Address: This authorization is made this day of , 20 by and between MasterDry, LLC, hereinafter referred to as MasterDry, LLC and (CUSTOMER), for the property located above. Customer acknowledges that when emergency conditions exist at the Work Site and that Customer has accordingly requested that work be commenced on an expedited basis without the delay involved in preparation of an estimate. MasterDry, LLC shall charge, and Customer agrees to pay, MasterDry, LLC’s standard prices for the services and materials actually provided.
