THIS CONTRACT is made and entered into by and between the Georgia Department of Community Health (hereinafter referred to as “DCH” or the “Department”) and XXXX (hereinafter referred to as the “Contractor”) and shall be effective on the date signed by the DCH Commissioner after receipt of any approval required by the Centers for Medicare and Medicaid Services (hereinafter referred to as “CMS”) (hereinafter referred to as the “Effective Date”).
