I, the authorised signatory, warrant that I am duly authorised to sign this Debit Order Authorisation Agreement and by my signature hereto, confirm that I shall have no claims against Focus on Health (Pty) Ltd arising out of this debit order authorisation. I understand that in the event that any debit order due in terms of this Debit Order Authorisation is rejected by my bank, all related bank rejection fees and the full amount of the initial balance that remains unpaid, will be due and payable immediately. An administration fee of R10 per rejected debit order will be charged on the account of the subscriber.
