The Health and Human Services Commission (HHSC) and Bluebonnet Trails Community MHMR Center d\b\a Bluebonnet Trails Community Services (Grantee), a Community Mental Health Center established under Texas Health and Safety Code Section 534.001, each referred to herein as a “Party” and collectively as the “Parties,” to the grant agreement providing funding to support service expansion in relation to the COVID-19 Supplemental Grant Program effective October 22, 2021 and denominated HHSC Contract No. HHS001108400005 (“Contract”), now desire to amend the Contract.
