THIS SUPPORT AGREEMENT, including all exhibits and schedules, is made and entered into by and between MANAGED ACCESS TO CHILD HEALTH, INC. d/b/a/ PARTNERSHIP FOR CHILD HEALTH (“THE PARTNERSHIP” herein) and Daniel Memorial, Inc. (“AGENCY” herein), and is made effective as of June 15, 2020 for the purpose of engaging one qualified Clinician to work as part of the Sulzbacher Village multidisciplinary medical-behavioral health team providing services related to the Substance Abuse and Mental Health Services Administration (SAMHSA), Expansion Grant–System of Care Initiative Program.
