THIS AGREEMENT made and entered into by and between the COUNTY OF HENNEPIN, STATE OF MINNESOTA, hereinafter referred to as the “COUNTY,” on behalf of the Hennepin County Human Services and Public Health Department, A-2300 Government Center, Minneapolis, Minnesota 55487, hereinafter referred to as the “DEPARTMENT” and Medical Transportation Management, Inc. (an ‘S’ corporation duly organized under the laws of the State of Missouri), 16 Hawk Ridge Drive, Lake Saint Louis, Missouri 63367, hereinafter referred to as “CONTRACTOR.”
