IN CONSIDERATION FOR (a person 18 years of age or older, referred to herein as the “Athlete”) receiving permission and being allowed to utilize the athletic facilities, equipment and premises of the Parish Episcopal School of The Church of the Transfiguration, I/we hereby RELEASE, WAIVE, DISCHARGE AND COVENANT NOT TO SUE the Parish Episcopal School of The Church of the Transfiguration, their officers, servants, agents, employees, volunteers, and board of trustees (hereinafter referred to as “Releasees”) from any and all liability, claims, demands, actions or causes of action of any kind arising or related to any loss, damage or injury, paralysis, including death, that may be sustained by “Athlete” WHETHER CAUSED BY THE NEGLIGENCE, WRONGFUL ACTS, OMISSIONS OR PREMISES DEFECT OF THE RELEASEES.
