Please be aware that this agreement is a contract that requires a $150 non-refundable deposit to reserve your date. Cancellations for any and all services listed on this contract (including bridal party services) must be made two weeks prior to the wedding day. Please note that booking is first come first serve, so return this form quickly to secure your date.
