(“LBS”), a New York Corporation with a principal place of business at 333 Butternut Drive, Syracuse, NY 13214 and Elmira Heights Central School District (hereinafter referred to as the “Employer”), with a principal place of business at 2083 College Avenue, Elmira Heights, NY 14903 concerning the Employer’s welfare benefit plan (the “Plan”). This Agreement is effective as of July 1, 2018 (the “Effective Date”).
