The Health and Human Services Commission (“HHSC” or “System Agency”), and Bluebonnet Trails Community MHMR Center (“Local Government” or “Grantee”), a Community Mental Health Center as defined by Texas Health and Safety Code § 534.001, each a “Party” and collectively the “Parties,” enter into the following grant contract for operation of a Hospital Transition Pilot Program designed to step-down/transition individuals who are psychiatrically and/or medically fragile from inpatient state hospital settings to more appropriate community- based settings (the “Contract”) pursuant to the provisions of the “Interlocal Cooperation Act,” Texas Government Code Chapter 791.
