For and in consideration of the undersigned participant's in activities sponsored by the Town of North Collins and/or North Collins Central School (hereinafter "Sponsors"), the undersigned does hereby relinquish any and all claims for the cause of action for personal injury, property damage or wrongful death occurring to the participant arising out of participation in the activities sponsored by the Sponsors and/or any activities incidental thereto, whenever or however they may occur and for such period said activities may continue, and by this agreement any such claims, rights, and causes of action that participant may have are hereby relinquished and the participant (or their parent(s), guardian(s), does (do) so on behalf of my/our child and participant's heirs, executors, administrators and assigns.
