City of Tempe (Plan Sponsor) has entered into a Master Services Agreement dated November 6, 2009 with Administration Resources Corporation, dba OptumHealth Financial Services (Plan Supervisor) for the purpose of providing certain employee health care benefit services. This document contains a detailed description of the services to be provided by Plan Supervisor as well as the corresponding fee(s) for providing said services.
