I, the undersigned, am the Participant named above. I will participate in the Program as the chaperone for my child(ren) and/or for child(ren) that have been specifically entrusted to my care by parent(s)/guardian(s) of other children participating in the Program. I understand that, as a chaperone, it is my responsibility to actively and at all times supervise my children and any children who have been entrusted to my care. The University of Utah prohibits participation in the Program by any individual who has engaged in any criminal activity of a sexual or violent nature. By signing this agreement, I acknowledge and agree that I have never engaged in, been charged with, or been convicted of, a crime of a sexual and/or violent nature.
