THIS AFFILIATION AGREEMENT is made this day of , 20 , by and between the FLORIDA GOLF COURSE SUPERINTENDENTS ASSOCIATION, a nonprofit corporation (FGCSA) and , a nonprofit corporation (hereinafter referred to as Chapter).
