THIS AGREEMENT is entered into by and between (hereinafter, “the Healthcare Entity”) and the State of Indiana acting by and through the Indiana Professional Licensing Agency on behalf of the Indiana Board of Pharmacy, State of Indiana (hereinafter, the Agency”). The Healthcare Entity and the Agency are hereinafter collectively referred to as “the Parties.”
