This form must be completed and signed by both the internship supervisor and student intern, and then emailed for departmental approval to anthro@dornsife.usc.edu and zeamer@usc.edu. Once this form has been reviewed, the student will receive approval (D-clearance) to enroll in ANTH 393 Directed Internship.
