This Business Associate Agreement (“Agreement”) is entered into as of (“Effective Date”) by and between the Yakima Valley Farm Workers Clinic, a Washington not-for-profit corporation (“Covered Entity”) and the undersigned (“Business Associate”). Each of Covered Entity and Business Associate may be referenced in this Agreement as a “Party” and collectively as the “Parties.”
