This Agreement is made and entered into by and between the Tacoma-Pierce County Health Department, hereinafter referred to as DEPARTMENT, and SAMPLE DEPARTMENT OF HEALTH hereinafter referred to as the HEALTH JURISDICTION. The DEPARTMENT and the HEALTH JURISDICTION are collectively referred to as the “parties.”
